N2  -  Clostridium difficile infections (CDI) represent an important nosocomial disease, mainly related to antibiotic use and hand hygiene (HH). The objective of this study was to evaluate the incidence of CDI at the Istituti Ospitalieri di Cremona, in the period 2000-2012, and factors associated with CDI. Materials and methods. We evaluated the incidence of CDI, adherence to HH, and antibiotic use in the whole hospital and in the internal medicine/haematology (IMH) wards. The Pearson correlation coefficient was calculated to identify risk factors associated with CDI. Results. Four hundred and 92 cases of CDI, diagnosed in 457 patients, have been observed. The incidence rate varied between 0.2/1000 admissions in 2004 to 4.52 in 2012. Ninety percent of cases have bee n diagnosed in 7 wards and over 2two thirds in 3 units: IMH, rehabilitation medicine, and infectious diseases. We observed a correlation between CDI and carbapenem use (r2= 0,55). In IMH a correlation was observed between CDI and 3/4 generation cefalosporins (C3/4G) (r2= 0,6) and carbapenems (r2= 0,55). No correlation was observed between HH and CDI. Discussion. We observed a striking increase in CDI incidence during the study period. A correlation between CDI and use of carbapenems and C3/4G was observed, this last in IMH only. No correlation  between HH and CDI was observed. To control CDI we think that it is important that every hospital will start antimicrobial stewardship programs focused particularly on the use of C3/4G and carbapenems.
