N2 - Summary Background. In 2009, WHO launched a multimodal strategy and campaign to improve hand hygiene practices worldwide; then in 2010 WHO invited to complete the “Hand Hygiene Self-Assessment Framework” (HHSAF); this is a validated tool based on the WHO multimodal strategy to evaluate the level of application of the Multimodal Hand Hygiene Improvement Strategy (MHHIS), which defines preventive interventions, standards and tools conceived to improve hand hygiene. Since 2014, in line with the guidelines promoted within the multimodal strategy, the reporting of HHSAF for self-assessment has been inserted among the Piedmont regional indicators. The aim of our study is to evaluate the implementation level of multi-modal strategy in Piedmont healthcare units in 2014, using the HHSAF document. Methods. Our surveillance was carried out through collection and analysis of 50 Piedmont healthcare facilities data, recorded through the HHSAF from January till December 2014. HHSAF describe hand hygiene level through evaluation of 5 parameters: system changes, education and staff training, evaluation and feedback, reminders in the workplace and promotion of an institutional safety climate. Findings. We observed that 70% of Piedmont healthcare facilities involved in the study achieved the intermediate hand hygiene level, 19% of them the advanced level and 11% the basic level. No one showed inadequate level of WHO multi-modal implementation. Our results show some critical issues in direct and indirect monitoring of hand hygiene, in providing reminders and in the active involvement of patients, family members and caregivers. Moreover, we observed the lack of events dedicated to World Day of Hand Hygiene (5 th May). Conclusion. The implementation of the multi-modal strategy obtained all over the world important results in terms of hand hygiene. In Piedmont hand hygiene reached an overall good level, in particular in supply and availability of hand washing products and staff education and training. HHSAF can facilitate and make homogeneous and comparable the analysis of the health facilities performance; at the regional level, its use may represent for the institutions a tool to measure and compare the results, the attitudes and the performance of the health facilities over time, and to design local strategies improving the program for hand hygiene.
